Step-by-Step Guide for Reviewing Customer Notification Certification Forms

Access the Submissions Tab
- To begin, log in to your State Dashboard and navigate to the Submissions tab.
- In the Submissions table, you'll see columns related to customer notices. These include:
- The Customer Notice column, which shows the status of the certification forms.
- A Certification Completion Date column.
- A link to view the submitted responses.

Filtering for Submitted Forms
- To simplify the review process, create a saved view to focus on only the submitted customer notice certification forms to narrow down on exactly which forms need review.

Reviewing Certification Forms

Click on a specific submission in the table to view its details.
- For example, if a form was completed on October 18th, you can click the file icon next to the form to review all submitted details.
- You can also download any examples of notices that were attached for review.

- Accepting or Rejecting a Certification Form

- If the form and attached documents meet all requirements:
- Change the form’s status to Accepted.
- Select the Accept Customer Notice Submission option.
- This will update the water system’s PWS account to show that the certification form has been accepted.

- Select Reject and confirm your decision by clicking the Reject button.
- The water system’s PWS account will be updated to show that the form has been rejected.
